Root Canals with HPSM Medi-Cal: A Millbrae Guide

Root Canals with HPSM Medi-Cal: A Millbrae Guide

· Carlmont Dental Care

Signs an infected tooth may need a root canal, what the treatment involves, and how Millbrae HPSM Medi-Cal members can confirm dental coverage details before booking.

A root canal removes the infected or damaged inner tissue of a tooth so you can keep the tooth instead of losing it. Persistent pain, swelling, or lingering temperature sensitivity are common warning signs. If you are an HPSM member in Millbrae, treatment may be covered, but benefit details vary, so confirm them with HPSM Dental before your visit.

How do I know if an infected tooth needs a root canal?

The pulp is the soft tissue inside a tooth, and decay, cracks, repeated dental work, or an injury can inflame or infect it. Only a dentist can confirm the diagnosis with an exam and X-rays. Still, some signs are worth taking seriously:

  • Throbbing or persistent tooth pain, often worse when you bite or chew
  • Sensitivity to hot or cold that lingers after the drink or food is gone
  • Swelling in the gums or face near the tooth, or a small bump on the gum
  • A tooth that has darkened or turned gray
  • A deep cavity, or a cracked or chipped tooth

Sometimes an injured tooth causes trouble even when it looks fine on the outside. Pain, swelling, or a bump on the gum is a good reason to call promptly rather than wait for it to settle on its own. Infection that spreads can become much harder to treat.

What does a root canal involve?

Root canal treatment is usually finished in one or two visits and is done with local anesthesia, so the area is numb. A dentist will typically follow these steps:

  1. Take X-rays and numb the tooth and the tissue around it.
  2. Isolate the tooth with a thin protective sheet to keep it clean.
  3. Make a small opening and remove the infected or damaged pulp from the chamber and the root canals.
  4. Clean the canals, sometimes with an antimicrobial medication, and seal them with a rubber-like filling material.
  5. Place a temporary filling until the permanent restoration is ready.

Afterward, mild tenderness for a few days is common, and a dentist can suggest ways to ease it. If the infection had spread, antibiotics may be prescribed. A follow-up visit usually replaces the temporary filling with a permanent filling or a crown. Dentists often recommend a crown to protect a treated tooth from fracturing. With good daily brushing, cleaning between teeth, and regular checkups, a restored tooth can last for many years.

How does HPSM Medi-Cal dental fit in?

Our office accepts Medi-Cal dental through Health Plan of San Mateo and is an HPSM-partnered provider. You can read more about Medi-Cal dental care through HPSM. HPSM dental benefits are generally described as covering preventive care (exams, cleanings, X-rays), restorative treatment such as fillings and other necessary procedures, and emergency care for pain or injury. Whether a root canal is covered, and any limits on frequency or on the tooth involved, can vary. For that reason, it helps to ask HPSM directly rather than guess.

Eligibility rules can also change. HPSM has announced dental benefit changes that affect some members starting in 2027, depending on their circumstances. A short call can tell you exactly where you stand today.

What should Millbrae members know before calling us?

We hear from many Millbrae families, from Millbrae Meadows to Mills Estates to the blocks around downtown Broadway, who aren't sure what their coverage includes or where to start with a painful tooth. Our practice takes a limited number of Medi-Cal patients from San Mateo County, so please call (650) 591-1984 and ask about current availability before assuming an appointment is open. We can't promise acceptance, but we can tell you honestly where things stand.

When you call, have your HPSM member card ready, and bring it to your visit along with a photo ID. Describe your symptoms, including when the pain started and whether you've noticed swelling. If you are in significant pain or have facial swelling, say so right away, since emergency care for pain is part of the general benefit categories. Patients coming from Millbrae typically find it about 12 to 18 minutes south to our Belmont office via Highway 101, El Camino Real, or I-280, depending on traffic.

Common questions about root canals and HPSM coverage

Q: Does a root canal hurt?

The tooth is numbed with local anesthesia, so most people feel pressure rather than sharp pain during treatment. Some tenderness for a few days afterward is common and usually manageable.

Q: Is a root canal covered by HPSM Medi-Cal dental?

It may be, but coverage can depend on the tooth involved and on plan limits. Ask the HPSM Dental Unit at 650-616-1522 or check your member materials to confirm before treatment.

Q: Can I wait if the pain goes away?

Not necessarily. Pain can ease when the nerve inside the tooth dies, while the infection remains. It is best to have the tooth examined even if it seems to have quieted down.

Q: Will I need a crown afterward?

Often, yes, especially for back teeth that handle heavy chewing. A dentist will explain whether a filling or a crown makes sense for your tooth.

Q: Do you accept every HPSM member?

We see a limited number of Medi-Cal patients from San Mateo County, so availability changes. Call us to ask.

★★★★★

“Took my kid to see Dr. Jiang today because his baby tooth wouldn’t fall out, and the new one was already coming in. He was super nervous, but Dr. Jiang was so patient and kind. They even had cartoons for him to watch! In no time at all, the tooth was out—painless! My son said it didn’t hurt at all. Awesome experience! Highly recommend Carlmont Dental!”

“I was referred to Dr. Jiang by a good friend of mine (I was having terrible luck finding a Dentist). First may I say I was very impressed upon arrival. Not only was the staff super friendly and helpful checking in as a first time patient, but the office itself was super clean and well maintained.”
