Tooth Extractions on HPSM Medi-Cal: San Bruno Patient Guide

Tooth Extractions on HPSM Medi-Cal: San Bruno Patient Guide

· Carlmont Dental Care

Facing a tooth that can't be saved? Learn how extractions work, how to heal well, and how to ask HPSM about replacement options. A guide for San Bruno members.

When a tooth is too damaged or infected to repair, a dentist may recommend removing it, and HPSM Dental may cover extractions that are medically necessary. Healing usually goes smoothly with a few simple habits, and it's worth asking about replacement options and coverage before or soon after the tooth comes out. This guide is for HPSM members in San Bruno and nearby communities. It is general education, not a substitute for an exam and advice from a dentist.

When can't a tooth be saved?

Dentists try to keep natural teeth whenever it's reasonable. Still, some situations make removal the safer choice: a crack that runs below the gum line, decay that has destroyed most of the tooth, advanced gum disease that has loosened it, or an infection that can't be resolved another way. An exam and X-rays help a dentist explain what's happening and whether alternatives such as a filling, root canal, or crown are realistic.

Many people wait, hoping a painful tooth will settle down. Pain that fades can still mean an infection is present, so it's worth having it checked.

Seek emergency care right away for swelling that affects breathing or swallowing, uncontrolled bleeding, or a high fever.

How do I use my HPSM Dental benefit for an extraction?

Medi-Cal dental through Health Plan of San Mateo generally includes preventive care (exams, cleanings, X-rays), restorative treatment such as fillings and other necessary procedures, and emergency care for pain or injury. Exactly what is covered, how often, and which advanced treatments are included can vary, so confirm details with the HPSM Dental Unit at 650-616-1522 or in your member materials. You can read more about Medi-Cal dental care through HPSM on our site.

Our office accepts Medi-Cal dental through HPSM for a limited number of San Mateo County patients. Please call (650) 591-1984 to ask about current availability before assuming an appointment is open. If you're coming from San Bruno, it's roughly a 15 to 20 minute drive, depending on traffic, south to our Belmont office via I-280, Highway 101, or El Camino Real.

What to bring to a visit:

  • Your HPSM member card and any other insurance or Medi-Cal cards you have (call the office if you're unsure what to bring)
  • A photo ID
  • A list of your medications and any health conditions
  • Notes on when the pain started and what makes it better or worse

What happens during and after an extraction?

A dentist will typically numb the area, loosen the tooth, and remove it. You typically feel pressure rather than sharp pain; tell the dentist if you feel pain during the procedure. A tooth that is broken or impacted may need a more involved approach, which is something to discuss at the consultation.

Once the tooth is out, a blood clot forms in the socket and protects the bone and nerves underneath. Most aftercare is about keeping that clot in place, and you should follow the aftercare instructions your dentist gives you:

  • Take pain relievers and any prescribed antibiotics exactly as directed.
  • Hold a cold pack against the outside of your cheek to ease swelling.
  • Avoid straws, forceful spitting, and smoking or tobacco, since suction and smoke can dislodge the clot.
  • After the first 24 hours, rinse gently with warm salt water rather than swishing hard, and skip brushing right at the site for the first day.
  • Stick to soft foods like yogurt, applesauce, and thin soups for a few days, and avoid crunchy or sticky foods and small bits like rice for a few weeks.

Call the office if pain gets worse several days after the extraction instead of easing, if it spreads toward your ear or eye, or if you notice a bad taste or bad breath. Those can be signs of dry socket, which happens when the clot is lost early and is most common after lower molar extractions. A fever may point to infection. Dry socket is treatable, and a dentist can clean the area and place a medicated dressing to calm it down.

What about replacing the tooth?

Leaving a gap can allow neighboring teeth to drift and can change how you chew over time. Dentists commonly discuss options such as a dental implant, a bridge, or a partial or full denture, depending on the location of the missing tooth, the health of the surrounding teeth and bone, and your goals.

Coverage for replacement treatment is where HPSM benefits can differ the most. Some options may be covered and others may not, and certain treatments can require prior approval. It helps to raise the question early: ask the dental team what they would recommend, then ask HPSM what is covered for that specific option. If you're a San Bruno resident deciding between choices, that conversation is worth having before the extraction, not after.

Common questions about extractions and HPSM Medi-Cal

Q: Is an extraction covered by HPSM Dental?

Extractions that are medically necessary may be covered. HPSM Dental generally covers emergency dental services, but coverage details vary, so confirm with the HPSM Dental Unit at 650-616-1522 or your member materials.

Q: Can I be seen right away for a painful tooth?

Our office takes a limited number of Medi-Cal patients from San Mateo County, so availability changes. Call (650) 591-1984 and describe your symptoms so the team can tell you what's possible. For swelling that affects breathing or swallowing, uncontrolled bleeding, or a high fever, seek emergency care right away.

Q: How long does healing take?

The first few days are the most tender, and the gum surface usually closes over in the following weeks. Complete bone healing takes longer, which is one reason replacement planning is often discussed with a dentist soon after removal.

Q: Do I have to replace a missing tooth?

It isn't an emergency, but leaving a gap can lead to shifting and bite changes. A dentist can help you weigh the options and what your plan may cover.

“It was suggested to remove my last molar since it was loose and no tooth above. I agreed. Two weeks later I show up for appointment and I’m presented with cost analyses. Two thousand for extraction, insurance covering 1200 and I’m required to pay 800 up front.”
