Gum Disease and Deep Cleanings on HPSM Medi-Cal: South San Francisco

· Carlmont Dental Care

Bleeding gums? Learn how gum disease is treated, what a deep cleaning (scaling and root planing) involves, and what to ask HPSM about Medi-Cal dental coverage in South San Francisco.

Bleeding gums can be an early sign of gum disease, and early gum disease can usually be reversed with good home care and professional cleanings. If it has advanced, dentists commonly recommend a deep cleaning called scaling and root planing. HPSM Medi-Cal members in South San Francisco can ask whether this treatment may be covered, and how often, by calling the HPSM Dental Unit.

This article is general education and not a diagnosis or individual medical advice. Only a dentist can diagnose gum disease after an exam.

Why do my gums bleed, and is it gum disease?

Healthy gums are firm and pink, and they do not bleed with normal brushing. Gums that are red, swollen, or bleed easily can be a sign of gingivitis, among other causes. Gingivitis is the mild, early stage of gum disease. At this stage the problem is typically confined to the gums, and it can often be reversed with daily brushing, flossing, and regular cleanings by a dentist or hygienist.

If gingivitis is left alone, it can progress to periodontitis. Here the gums pull away from the teeth and form pockets that can become infected. Over time, the bone and tissue that hold teeth in place can break down, and teeth may loosen or be lost. Because early gum disease is often painless, many people first notice only a little pink on the toothbrush or floss. That small sign is worth a checkup.

Signs worth getting checked

  • Gums that bleed when you brush or floss
  • Red, puffy, or tender gums
  • Persistent bad breath or a bad taste
  • Gums that look like they are receding
  • Teeth that feel loose or have shifted

What is a deep cleaning (scaling and root planing)?

A routine cleaning focuses on the visible surfaces of the teeth and just beneath the gumline. A deep cleaning goes further. Scaling removes plaque and hardened tartar from the tooth surfaces and from inside the gum pockets. Root planing smooths the root surfaces, which helps the gums heal and may help them reattach. The work is done with ultrasonic instruments and fine hand tools, and the area is often numbed for comfort.

Dentists and periodontists commonly use scaling and root planing as the first treatment step for periodontitis. It is sometimes done in more than one visit, and a dentist will usually schedule a follow-up to check whether the pockets are shrinking and the gums are healing. After treatment, people with gum disease generally need more frequent maintenance visits and consistent daily flossing to keep pockets from returning.

What should I ask HPSM about coverage?

San Mateo County Medi-Cal members receive their dental benefits through Health Plan of San Mateo (HPSM Dental). Medi-Cal dental benefits can include preventive, restorative and emergency care, but covered services and limits change, so confirm with HPSM. Ask HPSM which card to bring. Gum treatment may be covered when it is considered necessary, but benefit limits and frequencies vary, so it is best to confirm details before treatment.

Good questions to ask the HPSM Dental Unit at 650-616-1522, or to look up in your member materials:

  • Is scaling and root planing a covered benefit for my situation?
  • Does it require X-rays, gum measurements, or prior authorization?
  • How often can it be covered, and does it apply to the whole mouth or only certain areas?
  • What follow-up or periodontal maintenance visits are covered after treatment, and how often?

Our office can walk through Medi-Cal dental care through HPSM with you, but the plan itself is the final word on what it covers.

Common questions about gum disease and Medi-Cal

Q: Is a deep cleaning the same as a regular cleaning?

No. A regular cleaning is preventive and works mostly on the visible tooth surfaces. A deep cleaning treats the area below the gumline and the root surfaces when gum disease has developed. They are typically billed as separate services, so coverage can differ.

Q: Can gingivitis be reversed?

Usually, yes. Daily brushing and flossing plus regular professional cleanings can often return the gums to health when the problem is caught early. Periodontitis can usually be controlled with treatment and ongoing maintenance, but bone that has already been lost generally does not grow back without specialized procedures, which is why early evaluation helps.

Q: Does HPSM cover deep cleanings?

They may be covered when a dentist determines they are necessary, but limits and requirements vary. Confirm with the HPSM Dental Unit at 650-616-1522 or check your member materials.

Q: Does a deep cleaning hurt?

The area is often numbed, so most people feel pressure rather than pain. Gums can be tender for a few days afterward, and a dentist will explain how to care for them.

“I was referred by my friend and this is my third time here for the cleaning. This time, my dental hygienist is Lu Lin. She is very professional and full of patience, answered all of my questions. I always grind teeth at night and am not sure if this will cause the issue in the future. They provided suggestions and make the plan for me.”
